Նոր սերնդի գենետիկական վերլուծությունը կարող է օգնել կանխատեսել սրտի ռիթմի խանգարումները

ԱՄՆ-ի Հյուսիսարևմտյան համալսարանի գիտնականները մշակել են ռիսկերի գենետիկական գնահատման նոր մեթոդ, որը կարող է ավելի ճշգրիտ կանխատեսել առիթմիայի զարգացումը. սա սրտի ռիթմի խանգարում է, որը կարող է հանգեցնել լուրջ վիճակների, ինչպիսիք են ատրիալ ֆիբրիլյացիան կամ սրտի հանկարծակի մահը։

Հետազոտությանը մասնակցել է 1119 մասնակից։ Արդյունքները հրապարակվել են Cell Reports Medicine ամսագրում։

Այս մոտեցման նորությունն այն է, որ հետազոտողները միավորել են գենետիկական վերլուծության երեք տեսակ՝ առանձին գեների հազվագյուտ մուտացիաները (մոնոգեն տարբերակներ), բազմաթիվ տարածված գենետիկական տարբերակները (պոլիգեն տարբերակները) և գենոմի ամբողջական սեքվենավորման տվյալները (ներառյալ ոչ կոդավորող հատվածները)։

Սովորաբար գենետիկական թեստերը կամ ստուգում են միայն մեկ գեն, կամ կազմում են բազմագենային միավոր, կամ կարդում են ամբողջ գենոմը, սակայն հազվադեպ են անում ամեն ինչ միասին։ Հեղինակները նշում են, որ «Մենք համատեղում ենք հազվագյուտ և սովորական գենետիկական տարբերակները և ավելացնում գենոմի ոչ կոդավորող հատվածները։ Որքանով մեզ հայտնի է, մինչ այս ոչ ոք չի օգտագործել նման համալիր մոտեցում»։

Աշխատանքի ընթացքում գիտնականները վերլուծել են 523 մասնակցի՝ առիթմիայով (որոշները նաև սրտային անբավարարությամբ) և 596 վերահսկիչ մասնակցի NUgene բիոբանկից (տարիքը 40+ տարի, առանց սրտային հիվանդությունների պատմության)։ Նրանք համեմատել են գենոմները, հաշվարկել ռիսկի միավորը և համոզվել, որ իրենց մեթոդն իսկապես թույլ է տալիս նույնականացնել այն մարդկանց, ովքեր առիթմիայի զարգացման ամենամեծ հավանականությունն ունեն։

Հետազոտողների կարծիքով՝ արդյունքը կարող է կարևոր քայլ լինել այն բանի համար, որ բժիշկները կարողանան նախապես որոշել, թե ով ունի բարձր ռիսկ, նույնիսկ մինչև ախտանիշների ի հայտ գալը։ Սա հնարավորություն է տալիս կիրառել կանխարգելման և բուժման ավելի ճշգրիտ միջոցներ՝ օրինակ՝ դիտարկում, դեֆիբրիլյատորի տեղադրում կամ կենսակերպի փոփոխություն։

Գիտնականներն ընդգծում են, որ առայժմ գենետիկական թեստավորումը բավականաչափ լայնորեն չի կիրառվում։ Այն մարդկանց միայն 1–5%-ն է անցնում, որոնց համար այն ցուցված է։ Նրանք կարծում են, որ բազմագենային միավորների և համալիր վերլուծության զարգացման հետ մեկտեղ սրտաբանության և այլ ոլորտներում գենետիկական թեստավորման անհրաժեշտությունը միայն կաճի։
